The end-of-season play-offs will take place across two weekends:

Semi-Finals – 24th / 25th / 26th April
- One match only
Fixtures:
2nd vs 5th (2nd at home)
3rd vs 4th (3rd at home)

Final – 1st / 2nd / 3rd May
- One match only
- Hosted by the highest-placed semi-final winner

All ties are single-game deciders, with league position determining home advantage.
